Committee Resources - Committee Information and Forms

Evaluators participating in the off-site or on-site review of an institution undergoing reaffirmation of accreditation, in the review of a candidate institution, or the review of a member institution undergoing a substantive change will receive the Commission's Handbook for Review Committees. The following list provides access to forms used by the peer review committees of the Commission. Due to the recent adoption of the Principles of Accreditation and the implementation of a new review process, evaluators should always refer to this web site for the most updated forms used for the review of institutions.
